CAPACorrective and Preventive Action
CAPA stands for Corrective and Preventive Action. Corrective actions fix the root cause of a problem that has already occurred, such as an incident, audit finding, or failed inspection. Preventive actions address a potential problem before it happens. A working CAPA process assigns every action an owner and a due date, then verifies the fix was effective.

DARTDays Away, Restricted, or Transferred
DART stands for Days Away, Restricted, or Transferred. It is an OSHA rate that counts only the more serious recordable cases: those where an employee misses work, is placed on restricted duty, or is transferred to another job because of a work-related injury or illness. DART is calculated as DART cases times 200,000, divided by total hours worked.

EMRExperience Modification Rate
EMR, or Experience Modification Rate, is an insurance metric that compares a company's workers' compensation claims history to other companies of similar size in the same industry. An EMR of 1.0 is the industry average. A rate above 1.0 increases workers' compensation premiums and a rate below 1.0 reduces them. A single claim typically affects the EMR for three years.

JHAJob Hazard Analysis
A Job Hazard Analysis breaks a specific job into its individual steps, identifies the hazards associated with each step, and defines controls that eliminate or reduce them. OSHA recommends prioritizing jobs with high injury rates, new processes, or the potential for severe harm. A completed JHA becomes the basis for training and safe work procedures.

JSAJob Safety Analysis
A Job Safety Analysis uses the same core method as a Job Hazard Analysis: break a task into sequential steps, identify the hazards at each step, and assign a control to each hazard. The two terms are interchangeable in most industries, with JSA more common in construction and field service. Many contractors require a signed JSA before high-risk or nonroutine tasks begin.

LOTO, or Lockout/Tagout, refers to the procedures that control hazardous energy during service and maintenance of machines and equipment. Governed by OSHA standard 29 CFR 1910.147, LOTO requires isolating every energy source and applying locks and tags so equipment cannot start up or release stored energy while employees are working on it.

Near miss
A near miss is an unplanned event that did not result in injury, illness, or damage but had the potential to do so. Only a fortunate break in the chain of events prevented the harm. Near miss reporting matters because the same conditions, left uncorrected, tend to produce a real incident later. High reporting rates signal a healthy safety culture.

OSHA recordable
An OSHA recordable is a work-related injury or illness that must be entered on the OSHA 300 log. A Pre-Task Plan is a short planning document a crew completes before starting a task, usually at the work location at the start of the shift. SIFSerious Injury or Fatality
SIF stands for Serious Injury or Fatality. SIF programs classify incidents and near misses by their realistic worst-case outcome, not just what actually happened. Stop work authority is the explicit right and responsibility of any worker to halt a task, without fear of retaliation, when they believe conditions are unsafe. Toolbox talk
A toolbox talk is a short, informal safety meeting held with a work crew, usually at the start of a shift and typically lasting five to fifteen minutes. TRIRTotal Recordable Incident Rate
TRIR, or Total Recordable Incident Rate, measures how many OSHA recordable injuries and illnesses a company experiences per 100 full-time employees per year. The formula is recordable incidents times 200,000, divided by total hours worked. The 200,000 constant represents 100 employees working 40 hours a week for 50 weeks. Insurers and general contractors use TRIR to screen and benchmark companies.
